Bottled Fairy is an active item added in the initial release of Retribution.
Effects
- On use, fully heals all of Isaac's empty
Red Heart containers. - When Isaac would take fatal damage, this item is automatically used, preventing death.
Notes
- If Isaac has no Red Heart containers in their health bar when taking fatal damage, they will be given a filled Red Heart container.
- Bottled Fairy will not revive Isaac if he dies from spending all health at a Devil Deal, or from instant-death item effects such as from File:Suicide King.png Suicide King,
Plan C, and 
Damocles.
Interactions
Judas + 
Birthright: When activated, grants a permanent +3 damage boost, and converts all of Judas's Red Heart containers to Bone Hearts.
Tainted Bethany: Grants 12 blood charges when activated.
Trivia
- Bottled Fairy is a reference to items of a similar name in The Legend of Zelda series — in the various games, fairies collected by Link would prevent death.
