Death Cap is an active item added in the initial release of Retribution.
Effects
- When using for the first 5 times, Isaac takes damage and permanently gains the following modifiers:
- -0.35 damage.
- -0.06 speed.
- -0.06 shot speed.
- +5% chance to fire a poison tear.
- When using for the 6th time, Death Cap is destroyed, fully heals all of Isaac's
Red Heart containers and File:UI Bone Heart.png Bone Hearts, and grants the following modifiers:
- +2.75 damage.
- 1.5× damage multiplier.
- +0.5 speed.
- +0.5 shot speed.
Interactions
Teardrop Charm: Increases the chance of firing a poison tear.
The Forgotten / 
Berserk!: Death Cap's poison can be applied by Clubs.
Mom's Knife: Death Cap's poison can be applied by Mom's Knife.
Aquarius: Death Cap's poison can be applied by Aquarius.
Tainted Bethany: When the final stage of Death Cap is eaten, Tainted Bethany gains 12 blood charges.
Void: Absorbing Death Cap lets Isaac use the absorbed stage of the item, but the next stages will be permanently unavailable.

- Mithridatism is the act of gaining a resistance to poisons by gradually self-administering increasing larger non-lethal doses.
