Pumpkin Mask is an active item added in the initial release of Retribution.

Effects

• On use, grants 12 Soul Hearts. Afterwards, Isaac will occasionally take damage every 60 seconds and launch a Brimstone beam in the direction he is facing.

Interactions

• Book of Virtues: Activating Pumpkin Mask spawns an orange-blue wisp. This wisp spawns a Half Soul Heart every time Isaac takes damage from Pumpkin Mask's effect.
• Judas + Birthright: When Pumpkin Mask is used, grants 12 Black Hearts instead of Soul Hearts.

Trivia

• Pumpkin Mask is based on the cut item that was meant to appear in the Halloween update for the original The Binding of Isaac.
  • Pumpkin Mask was intended to be an item dropped by File:Krampus.png Krampus, and would have acted identically to File:Head of Krampus.png Head of Krampus.
