Joyful is a passive item added in the initial release of Retribution.
Effects
- When Isaac takes damage, he has a chance to gain a 2× damage multiplier for the room. In the next room afterwards, he will gain a 0.85× damage multiplier.
- 35% chance for
Joy to drop on room clear, alongside normal rewards.
- Joy pills instantly activate the effect of Joyful's damage multiplier when consumed. If Isaac consumes 3 or more Joy pills while he has cleared less than 10 rooms between each time a pill was consumed, he will gain the Addicted! pill effect in every room until this condition is no longer true.
Notes
- The 2× damage multiplier will stack with itself when Joyful's effect is activated multiple times, including that granted by the effects of using Joy pills.
Interactions
Empiric: Transforms all Joy pills into 
Euphoria pills, which grant double the effects of Joyful and the Addicted! pill effect when consumed. Consuming Euphoria pills will also count as using a Joy pill twice, when considering granting Isaac the prolonged Addicted! pill effect within a 10 room period.- File:Contract From Below.png Contract From Below: Joy pills that spawn on room clear have a 2/3 chance to be doubled, otherwise they will be removed. This effectively reduces the chance for Joy pills to spawn to ~23.33%.

- Joyful is a reference to LISA the Joyful.
- as well to the whole LISA series as joy can be seen in all of the fan games to some extent
- the scream whitch occurs on hit uses the same audio as the joy mutant scream from the LISA franchise
